Coordinator/Care Navigator

Mino Care is a holistic virtual perinatal health care centre. We offer access to a range of customized culturally safe health care services and supports to navigate reproductive and perinatal health.
In partnership with Parkdale Queen West CHC, Mino Care is looking for a Coordinator/Care Navigator for the refugee care program. The Coordinator/Care Navigator will serve as a liaison between patients, Mino Care and PQWCHC. The ideal candidate will have prior experience coordinating and advocating across integrated healthcare systems, as well as providing care navigation strategies.
● Collaborates with Parkdale Queen West CHC staff to prioritize refugees' perinatal healthcare needs
● Conducts intake assessments, needs assessments, treatment planning and reassessments to help refugees navigate the healthcare system
● Creates care plans and educates refugees and their families
● Manages the refugee's care plan, prioritizing health and psychological goals and providing necessary education and materials
● Reviews new information and complex cases with the care team and incorporate additional recommendations into the care plan
● Facilitates workshops at community health centres for prenatal classes once a week (virtually and in person)
● Conducts quality assurance and monitoring for service delivery and case reviews
● Performs other related duties incidental to the work described herein
● Current registration as a Registered Nurse with the College of Nurses of Ontario or current Certificate of Registration with the College of midwives in good standing
● 2 Years minimum of case management and health care experience
● Computer literacy is necessary
● Good communication and interpersonal skills to speak clearly with clients and other team members
● Demonstrates cultural competency and effective relationship-building skills with target population and team members
● Possesses First Aid and CPR certifications
Time Commitment:
Part-time- 18 hours a week
